Job Description

Immediate Need - Case Coord. and Intake Specialist for Home Health Care company



What is the Job?



Temp-Perm



Pay Rate Range 22-25/hr., based on experience



M-Fri 9am-5pm, but some shared on call duties evenings/weekends



This position monitors the on-time arrivals of Caregivers and re-routing and/or re-staffing of cases, if needed. Compassion, time management and attention to detail is a must.



Case Coordinator Responsibilities

1. Schedules home care cases, current and prospective

2. Proactively resolves staffing issues as a result of changes to client or caregiver's schedules

3. Records and maintains detailed data in scheduling logs and staffing calendars

4. Maintains record of absences (approved and unapproved) of all agency field personnel

5. Notifies management of all staffing problems, resolved and unresolved, in a timely manner

6. Responsible for the currency and accuracy of the scheduling calendars

7. Has shared on-call duties outside of regular work day which will include some evenings, weekend and holidays as assigned

8. Documents all occurrences during on-call shift and communicates change to the scheduling team via verbal and/or written communication after the end of each on-call shift

9. Processes clock ins and clock outs, and records data as needed for payroll processing

10. Maintains and respects confidentiality of client, employee, and agency information to ensure privacy is protected

11. Promote internal referral programs to attract new caregiver candidates

12. Schedule inactive caregiver calls or supervision of such



What is the Job?

Intake Coord. Temp-Perm M-Fri 9am-5pm 20-22/hr.

Will be calling leads to speak with potential clients/patients to speak about the companies home health care services

Spanish Bilingual a plus

Need to explain the services and make sure they are comfortable with the process

Prefer experience in home health care industry

Strong customer service experience required

Must be able to take potential clients/patients through the enrollment process if they would like to move forward and set up the services
